Request Form for OD Consulting

Requests will be reviewed three times a year. Indicate which submission deadline date you wish your application be reviewed.

Submission Deadline Date
	January 15		June 15		September 15

Priority will be given to requests by college deans who should initiate the request. Requests will also be accepted from other academic units.

Applicants will be notified of the outcome within two weeks of the review.

Selection Process

1. Applicant applies online at http://fod.msu.edu/odconsulting/form.asp

2. The Selection Committee (Theodore H. Curry II, Deborah DeZure, Allyn Shaw, and John Beck) will meet to discuss applications and select projects for funding.

3. A member of the Selection Committee will call applicants for clarification of needs as needed.

4. Replies will be sent within two weeks of application date.

5. Completion of project.

6. F&OD will send an evaluation form to be submitted to F&OD.
